[codex] Add issue subtree pause, cancel, and restore controls (#4332)

`0065_environments`.
- Added a small follow-up commit that makes restore requests return `200
OK` explicitly while keeping pause/cancel hold creation at `201
Created`, and updated the route test to match that contract.

## Risks

- This is a broad cross-layer change touching DB/schema, shared
contracts, server orchestration, and UI; regressions are most likely
around subtree status restoration or wake suppression/resume edge cases.
- The migration was renumbered during PR prep to avoid the new upstream
`0065_environments` conflict. Reviewers should confirm the final
`0066_issue_tree_holds` ordering is the only hold-related migration that
lands.
- The issue-tree restore endpoint now responds with `200` instead of
relying on implicit behavior, which is semantically better for a restore
operation but still changes an API detail that clients or tests could
have assumed.

b642d3e06b fix(adapter-utils): use cmd.exe for .cmd/.bat wrappers on Windows
Avoid relying on ComSpec for .cmd/.bat invocation in runChildProcess. Some Win11 environments set ComSpec to PowerShell, which breaks cmd-specific flags (/d /s /c) and causes adapter CLI discovery failures (e.g. opencode models).

c844ca1a40 Improve orphaned local heartbeat recovery
Persist child-process metadata for local adapter runs, keep detached runs alive when their pid still exists, queue a single automatic retry when the pid is confirmed dead, and clear detached warnings when the original run reports activity again.

9248881d42 fix(adapter-utils): strip Claude Code env vars from child processes
When the Paperclip server is started from within a Claude Code session
(e.g. `npx paperclipai run` in a Claude Code terminal), the `CLAUDECODE`
and related env vars (`CLAUDE_CODE_ENTRYPOINT`, `CLAUDE_CODE_SESSION`,
`CLAUDE_CODE_PARENT_SESSION`) leak into `process.env`. Since
`runChildProcess()` spreads `process.env` into the child environment,
every spawned `claude` CLI process inherits these vars and immediately
exits with: "Claude Code cannot be launched inside another Claude Code
session."

This is particularly disruptive for the `claude-local` adapter, where
every agent run spawns a `claude` child process. A single contaminated
server start (or cron job that inherits the env) silently breaks all
agent executions until the server is restarted in a clean environment.

The fix deletes the four known Claude Code nesting-guard env vars from
the merged environment before passing it to `spawn()`.

2c3c2cf724 feat: adapter model discovery, reasoning effort, and improved codex formatting
Add dynamic OpenAI model list fetching for codex adapter with caching,
async listModels interface, reasoning effort support for both claude and
codex adapters, optional timeouts (default to unlimited), wakeCommentId
context propagation, and richer codex stdout event parsing/formatting.

631c859b89 Move adapter implementations into shared workspace packages
Extract claude-local and codex-local adapter code from cli/server/ui
into packages/adapters/ and packages/adapter-utils/. CLI, server, and
UI now import shared adapter logic instead of duplicating it. Removes
~1100 lines of duplicated code across packages. Register new packages
in pnpm workspace.
